A child protection scheme for a weak teenager was wrongly removed, when she and her mother were found dead in their house, an inquiry has heard.
The body of 47 -year -old Alphonsin Djiako Leuga was discovered on 21 May 2024 in a house in Redford, Nottingham, his 18 -year -old daughter Lorin Cholla.
Concerns about Lorin were raised, who had Down syndrome and learning inability, and a child protection scheme was implemented by social care teams at Nottingham City Council in 2023.
On Wednesday, an inquiry about his deaths heard of removing the plan in January 2024, a “wrong decision”.
It is believed that alphonsin and lorin could die for weeks or months before they were found at their council house in Hartley Road, where they had been living since 2019.

The court heard that Lorin was “fully dependent” on alphonsin for eating and drinking and “primarily non-oral”, the court listened.
In 2021, Alphonsin stopped engaging with housing, education and social care services. By 2022, Lorin stopped attending a school with his special educational needs.
Giving evidence in Nottingham Koroner’s court, a service manager for the entire life disability team of Nicola God Council said that due to lack of his mother’s engagement, Lorin was considered a “weak child” and was known that his case was “complex”.
Despite concerns for Lorin, he added, there was no concern about his “presentation” and his relationship between him and his mother was seen as “warm”.
The court heard that “concerns about Lorin’s health continued”, his lack of education and his plan was abolished.
Lorin’s child protection scheme was closed on 31 January 2024, before she turned 18 years old.
Ms. Gode said from interrogation: “I think in the handicides, now that we have focused on it, this hair was a wrong decision made by social care. We could follow more intensive inquiry.”
He said: “We admit that we should not stop the plan”.
It was known for the social care team that Alphonsin was Seriously admitted to unhealthy hospital – Before her daughter’s plan ended – and “life -guard treatment” was required.
After her discharge, social care tried to travel to a house, but left when it was “no house”.
Ms. Godi said: “Did we believe that Alphonsin and Lorin were inside, we must have called the police.”
The inquiry heard after that day, on 3 February, Alphonsin argued for an ambulance on 999, but the call was mistakenly abandoned and later closed, in which no one was with participating in the property.
The proposed medical cause of alphonsin’s death was recorded as pneumonia, while lorine was not established.
